Sunset Home Policies:

1. Sunset Home will limit its services to hospice patients.

2. Patients will be referred to the Sunset Home Approval Committee by Angel of Shavano nurses

or the social worker.

3. A Sunset Home representative member will contact patient/family, make a home visit to discuss needs,

complete application form and financial assessment form.

4.  BEO Personal Care services of Salida will be contacted to provide the caregiving.

5.  All Patient information will be kept confidential and in accordance with HIPPA regulations.
